The first recorded plague pandemic, which erupted in Byzantine Egypt in 541, struck Constantinople at the height of Justinian's reconquest of the West, and returned in waves for two centuries. It is the opening chapter of humanity's long history with Yersinia pestis, more than a millennium before the Black Death.

The Plague Appears at Pelusium
The pandemic was first recorded at Pelusium, the port city at the mouth of the Nile's eastern branch, which handled grain shipments along the empire's dense trade network. The disease, bubonic plague caused by the bacterium Yersinia pestis, spread along shipping routes to Alexandria and then by grain fleet to Constantinople, demonstrating how the Roman Empire's own commercial arteries became pathways of contagion.

Constantinople Struck
The plague reached Constantinople in the spring of 542 and raged for about four months. The court historian Procopius recorded deaths rising to 5,000 a day at the peak and described the collapse of burial capacity, with bodies stacked in towers and shipped out by boat. Emperor Justinian I himself contracted the disease and recovered; by the epidemic's end roughly a fifth of the capital's population had died, by Procopius's account.

The Pandemic Spreads Across the Mediterranean World
The plague spread to every corner of Justinian's empire and beyond, striking the Sasanian Empire, Syria, North Africa, Italy, and Gaul by 543-544. Justinian's government responded with tax remissions for devastated provinces and efforts to secure grain supply, while the historian John of Ephesus, traveling through Syria, described towns emptied of inhabitants. The epidemic strained the resources Justinian needed for his wars of reconquest in Italy and North Africa.

The Plague of Rome and the Pontificate of Gregory the Great
A recurrence swept Rome in 590, killing Pope Pelagius II. His successor Gregory the Great, later credited with organizing prayer processions to end the outbreak, rose to the papacy amid the epidemic and led a Roman church confronting Lombard invasion, plague, and the collapse of imperial control in Italy, an era of crisis that shaped his papacy and writings.

Waves of Recurrence Through the Seventh Century
Unlike a single epidemic, the first plague pandemic returned in waves roughly every nine to fifteen years across the sixth and seventh centuries, striking cities from Spain to Mesopotamia. Contemporaries connected the pestilence to the upheavals of the age, including the early Islamic conquests, and later Arabic and Byzantine chroniclers recorded outbreaks through the 680s and 690s. Older estimates put the pandemic's cumulative toll at 25 to 50 million deaths.

The Great Outbreak of 746-747
The last major outbreak of the first pandemic devastated Constantinople in 746-747, by some chronicle accounts killing as many as 200,000 people in the capital and prompting the emperor Constantine V to repopulate the city by transferring people from the Aegean and Greek provinces. After this wave the pandemic burned out, and plague vanished from the Mediterranean record for six hundred years until the Black Death arrived in 1347.

Ancient DNA Confirms the Pathogen
Researchers recovered Yersinia pestis DNA from sixth-century plague victims buried at Aschheim in Bavaria, confirming that the Justinianic plague was indeed caused by the same bacterium as the Black Death. The genetic evidence suggested the strain emerged independently and later went extinct, rather than directly ancestor-ing later pandemic strains. The confirmation resolved the pathogen question while leaving the pandemic's full demographic impact contested.

The Debate Over the Plague's Impact
Beginning in the late 2010s, a scholarly revisionist movement challenged the traditional narrative of the plague as a civilization-shaking catastrophe, arguing that the evidence for tens of millions of deaths and a decisive blow to the Byzantine economy is thin and that many regions recovered quickly. Their critics, working with new paleoclimate and papyrological evidence, defended a high-mortality view.
