During a ferocious outbreak in Soho, the London physician John Snow mapped the deaths and found they clustered around the Broad Street water pump. By interviewing residents and removing the pump handle, and by showing that water company customers of one supplier died at far higher rates than their neighbors, Snow built the case that cholera spread through contaminated water — not bad air. His investigation, published the following year, founded modern epidemiology years before germs were identified.

In the hot summer of 1858, the Thames — then London's open sewer — stank so badly that Parliament debated fleeing the riverside. The German bacteriologist Robert Koch, working in Egypt and then Calcutta during the fifth pandemic, isolated and identified the bacterium Vibrio cholerae, establishing cholera's microbial cause. Koch's discovery settled the scientific argument Snow's data had started against the miasma theory, and moved epidemic control decisively toward bacteriology, water filtration, and disinfection.

The Seventh Pandemic Begins

A new biotype of the cholera bacterium, El Tor, emerged from Sulawesi in 1961 and began the seventh pandemic — the one still running today. It spread through Asia in the 1960s, reached Africa in the 1970s, and the Americas in 1991, hitting Peru's coastal cities hardest. Cholera's persistence in the modern era exposed the gap between the sanitation that had conquered the disease in wealthy countries and its absence among the world's poorest.

Oral Rehydration Therapy

Researchers in Dhaka and Kolkata demonstrated that a simple solution of sugar, salt, and clean water could keep cholera patients alive by replacing fluids faster than they were lost — reducing mortality from around 30 percent to under 1 percent in treated cases. Oral rehydration therapy, cheap enough for village use and requiring no refrigeration or needles, was later called one of the most important medical advances of the twentieth century and has saved tens of millions of lives from diarrheal disease.

Cholera Returns to Haiti

Ten months after the devastating January 2010 earthquake, cholera broke out in Haiti — the first case in the country's recorded history. Genomic and epidemiological studies traced the strain to UN peacekeepers from Nepal, whose base sewage had contaminated a tributary of the Artibonite River. The epidemic infected roughly 800,000 Haitians and killed about 10,000. The UN initially denied responsibility and claimed immunity from victims' legal claims, an episode that became a global case study in institutional accountability.

The UN Acknowledges Its Role

In December 2016, UN Secretary-General Ban Ki-moon publicly acknowledged the UN's role in introducing cholera to Haiti and apologized to the Haitian people — while stopping short of accepting legal liability. Victim compensation claims had been rejected on immunity grounds, and the promised aid package fell far short of what affected communities sought. The acknowledgment closed neither the epidemic's public health nor its legal aftermath, and it remains a defining example of the contested relationship between international institutions and the people harmed by their operations.
